Required Skills & Certifications:
- Active Registered Nurse license in the state of California (or pending)
- Minimum of 2 years of acute care Hospital Case Manager experience performing Care Coordination and Discharge planning
- Clinical Assessment, Medication Administration, and Treatment Planning skills
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelor's degree in Nursing or related field
- Driver's license, car insurance, and automobile for field work
