Home Health Physical Therapist – Full-Time

Our client is a locally owned, Medicare-certified home health agency that also accepts managed care and commercial insurance. They are seeking a dedicated Physical Therapist to join their growing team and deliver compassionate, high-quality care across the Jacksonville area.

Salary and Benefits

• Per-visit rate: $100 per Start of Care (SOC)
• $85 per Resumption of Care, Evaluation, Recertification, and Discharge
• Guaranteed minimum of 30 units per week (flexibility for more if desired)
• Mileage reimbursement at $0.40 per mile
• 4 weeks Paid Time Off (PTO)
• 7 Paid Holidays
• $1,000 Continuing Education (CEU) stipend every 2 years (must be job-related)
• Health insurance stipend covering up to 50% of premiums
• Vision and dental insurance available (employee-paid)
• No on-call requirement

Responsibilities

• Deliver physical therapy services to patients in their homes within Jacksonville limits (no travel to Palm Coast or Lake City)
• Perform Start of Care assessments, evaluations, recertifications, resumptions, and discharges
• Accurately complete OASIS documentation using Kinnser (WellSky) EMR
• Coordinate with the internal office team, which performs welcome calls and assigns patients
• Manage a caseload of 3–4 patients per day with clustered routing to minimize travel
• Meet Medicare’s 7-day requirement for Start of Care visits
• Support a patient population that is primarily Medicare-based, including two assisted living facilities
• Exercise full autonomy over weekly scheduling and visit grouping
• Communicate with interdisciplinary care team to promote optimal patient outcomes

Requirements

• Active and unrestricted Physical Therapist (PT) license in Florida
• Proficient in independently performing OASIS Start of Care assessments
• Prior home health experience strongly preferred
• Reliable transportation and ability to travel throughout Jacksonville and Yulee (minimal)
• EMR proficiency (Kinnser/WellSky preferred)
• Strong clinical judgment, organization, and communication skills
• New graduates will not be considered
• Visa sponsorship is not available
