Case Management Registered Nurse (RN) - Travel Position Location: Jackson, MS Job Type: Travel Contract Duration: 13 weeks Shift: 5x8s Days Pay: $2535/week Job Description: We are seeking an experienced and dedicated Case Management RN to join our team. In this role, you will play a vital part in ensuring patients receive seamless, high-quality care throughout their hospital stay and transition safely to the next level of care. The ideal candidate is detail-oriented, collaborative, and skilled in navigating complex discharge planning processes. Key Responsibilities: * Coordinate and manage individualized patient care plans from admission through discharge. * Conduct comprehensive assessments to identify clinical, psychosocial, and discharge needs. * Collaborate with physicians, social workers, and interdisciplinary teams to ensure safe and efficient transitions of care. * Advocate for patients and families, providing education and support regarding available resources and follow-up care. * Communicate with insurers, post-acute care providers, and other stakeholders to support continuity of care. * Ensure documentation and case records meet regulatory, accreditation, and facility standards.
